Registered Nurse - Program Coordinator / Community Liaison TrinityKids Care Hospice ( Full Time - Day Shift ) Bonus Eligible
Providence is calling a Registered Nurse / Community Liaison (Full time / Day shift) to Providence TrinityKids Care Hospice in Torrance, California
This position is eligible for a sign on bonus for external applicants.
In this position you will:
The Community Liaison for TrinityKids Care ( TKC ) performs patient evaluations and admissions including the interdisciplinary collaboration in initiating the Plan of Care for the patient/caregiver/family.
Collaborates with referral sources in the preparation and transition of patient/caregiver/family to hospice care services.
Required qualifications for this position include:
Graduate of an accredited School of Nursing
Minimum 1-year of recent RN experience in acute care, hospice, palliative care &/or home health, working with neonates/pediatric patient population
Current Califorina RN license
Current BLS for Healthcare Providers certification from the American Heart Association
Must have a valid driver's license with an automobile that is in good working order and insured in accordance with State and/or LCMHS/ Providence TrinityCare Hospice requirements.
Demonstrates ability to communicate clearly and effectively
Demonstrates working knowledge of computer applications, i.e. Microsoft Office and e-mail.
Preferred qualification for this position include:
Nursing experience in theory and practice in Hospice/Palliative Care or Home Health
Bilingual in Spanish a plus
